I understand that the programme is offered at no cost to the learners and
thus I do not expect to remunerated for my services.
I understand that sexual relations with ant student(s) is conflict of
interest and I consent to be take full responsibility for any criminal or
criminogenic charges that may be laid against me.
I further further understand that the institution will not be held liable
to any loss to or of my property while administering the srvices I offer,
nor will the entity be held responsible for any physical harm whether
inside or outside the premises of service.
I understand that programme has a religious background, namely a
Seventh-day Adventist background and promise to adhere to all their
policies and prescriptions including refraining from offering services on
Saturday which they refer to as the Sabbath.
I understand that that the entity may make use of or disclose my
information to the the SAPS in the event of an investigation in accordance
with the PAI Act.
Should such assistance such as a stipend be made available, I promise to
gratefully accept the amount as decided by any such board as responsible
for decision making as far as the amounts and the frequency of such
assistance is decided upon.
I consent to adhering to all the organisations rules and regulations as
well as their constitution.
